  • Insurance card (please bring current card to every visit)
  • Completed Financial Policy/Patient Consent (print form)
  • Credit card, HSA (Health Savings Account or HRA), or debit card (review/complete patient consent)                                                                         
  • Your child's immunization records (if you are transferring from another practice)
